Answers:

1) Residence certificate?

    - ID Card or original civil status record.

    - Land property or Rent certificate.

 

2-a) Individual civil status record (ICSR)?

    - ID card or original individual civil status record

    - 2 passports photographs

 

    2-b)Family civil status record?

    - ID card or individual civil status record of the concerned person

 

3) Birth certificate?

    - Certificate of the Hospital.

    - ID card or civil status record of both parents

 

 

4) Marriage certificate?

    - Marriage certificate

    - ID card or civil status record of the wedded

 

5) Death certificate?

    - Death attestation given by the priest or the Medical doctor or Hospital.

    - ID card or ICSR (Individual civil status record) of the deceased

    - ID card or ICSR (Individual civil status record) of the informer and that of a witness

 

6) Passport application form?

    - New ID card or a ICSR (Individual civil status record) dating not more than 3 months

    - ONE colored photograph with white background (4,3 cm ´ 3,5 cm).

    - Parents are asked to stand before the Mayor or the General Security office if the child would be traveling alone and his age is less then 7 years old. If he is older then 7 years, only the FATHER is requested to stand.

    - If the child is traveling with one of his parents, an approval of the parent not traveling is requested.

    - If one of the parents is abroad, his approval in an original copy certified by the Lebanese Embassy shall be sent via the post office (registered mail).

    - The presence of TWO witnesses with their ID card or ICSR (Individual civil status record) with the person requesting to travel.

 

7) Certificates, miscellaneous?

    - ID card or a ICSR (Individual civil status record) of the concerned person present with witnesses according to the type of certificate.

 

8) Request for a certified copy of a birth certificate?

    - ID card or ICSR (Individual civil status record) of the concerned person

 

9) Request for a certified copy of a marriage certificate?

    - ID card or ICSR (Individual civil status record) of the concerned person

 

10) Request for a certified copy of a death certificate?

    - ID card or ICSR (Individual civil status record) of the person requesting the certificate

    - The presence of the parents with their ID cards.
